Regarding Insurance: Please note that this office does not process insurance claims.  If you have insurance, we will provide you with a statement to submit to your insurance company and they will reimburse you,  if your plan covers our services.  Usually, you can expect to be reimbursed by your insurance company within a couple of weeks of submitting your statement.
