When hives continue beyond a couple of weeks, they are considered chronic. The raised, intensely itchy welts are caused by mast cells releasing histamine and other inflammatory chemicals into the skin. Antihistamines may quiet that reaction, which is appreciated for relief, but they do not answer the larger question: Why are the mast cells continuing to release histamine?
Chronic hives are often called “spontaneous” or “idiopathic” when no obvious allergen can be found. That does not mean there is no cause. It may mean the cause is not located where we are looking. The skin is displaying the problem, but the condition may involve many systems at once.
The digestive tract is an important place to start investigating. Poor digestion, intestinal irritation, altered gut bacteria, food sensitivities, parasites, lingering infections or poor microbiome may continually stimulate inflammation. Because much of the immune system communicates through the intestinal environment, ongoing digestive disruption can influence how reactive the entire body becomes.
The liver also matters. It helps process histamine, hormones, medications, environmental chemicals, and inflammatory compounds. When its workload is excessive or its pathways are poorly supported, the body may have greater difficulty clearing substances that contribute to itching and reactivity. Blood sugar instability and nutrient deficiencies can add another layer by reducing the energy available for tissue repair and normal cellular function.
The endocrine system should not be overlooked. Chronic hives have a well-established association with thyroid autoimmunity, sometimes even when standard thyroid hormone levels appear normal. Estrogen, progesterone, cortisol, and insulin can also affect inflammatory activity, histamine responses, and symptom timing. The adrenal glands and nervous system influence the body’s response to physical and emotional stress, which helps explain why poor sleep, exhaustion, or prolonged stress often intensify outbreaks.
Hidden viral, bacterial, dental, sinus, or other infections may also keep the immune system working overtime. Eventually, unresolved damage and repeated inflammation can develop into an autoimmune phase. It is a sign that the body has been responding to ongoing problems for too long and can no longer restore balance without help. Autoimmunity is a progression of a disease into an autoimmune stage.
To resolve hives, we have to look beyond the rash. We must evaluate digestion, including the microbiome, liver function, thyroid and hormonal patterns, blood sugar, nutrient status, stress physiology, environmental exposures, medications, and possible infections. Acupuncture can help calm itching and mast-cell reactivity while addressing nervous-system, digestive, endocrine, and immune functions, amongst others. Nutrition can then be directed toward the systems going awry.
The goal is not really to suppress each outbreak, although appreciated. It is to understand why the body keeps producing the hives in the first place and address the underlying reasons for that behavior.
